Abstract:

Background: Hemodialysis (HD) is the basic renal replacement therapy in Egypt, upon advancement to this final and irreversible stage of chronic kidney disease (CKD), patients encounter many stressors related to both nature of illness and life supporting treatments are often physically and psychologically exhausting for patients. Aim of work: to assess severity of depressive disorders for ESRD paints on regular hemodialysis in correlations with patient’s characteristics. Patients and methods: we conducted a cross section study to assess the prevalence and severity of depression among ESRD patients who are undergoing maintenance HD in Embaba hospital, Cairo, Egypt, using Beck’s depression inventory and PCASEE quality of life scale. Results: A total of 110 patients with mean age 54.4 ± 12.1 years were eligible for inclusion in the current study, our data showed that mean beck’s score for depression was 28± 17 and range 7-64. Mild depression was reported in 51.8%, moderate depression in 28.2%, and severe depression in 20%. Sensitivity analysis showed that creatinine, Calcium and Hemoglobin can significantly predict severe depression using a cutoff point 9 mg/dL and 8 mg/dl and 9 gm/dl respectively. Parathormone can significantly predict severe depression using a cutoff point 248 ng/dl. With sensitivity 90.9%, specificity 72.7% and p value 0.001. Conclusion: Depression was highly prevalent among HD patients in Egypt with severe depression affecting 20%, Calcium, hemoglobin, creatinine and parathormone levels can predict severe depression among patients with ESRD and under regular HD.
