Abstract:

Type 2 diabetes mellitus (Type 2 DM) is a metabolic disease characterized by hyperglycemia resulting form defects in insulin secretion, insulin action or both. Resistance to insulin characterizes type 2 diabetes mellitus, the most common form of diabetes mellitus. There is a strong link between type 2 diabetes mellitus and obesity, as most patients tend to be obese. Thiozolidinediones (TZDs), a new class of anti-diabetic drugs, enhances target-tissue sensitivity to insulin. A screen for genes down regulated by TZDs in adipocyte, led to the discovery of a new protein hormone called resistin (for resistance to insulin). Resistin, specifically produced and secreted by adipocytes, is present at elevated levels in the blood of obese animals, and is down regulated by fasting and anti-diabetic drugs. Antibody to resistin stimulated glucose uptake and improved insulin sensitivity in obese mice. The present study was performed on 80 individuals categorized as follow: Twenty apparently healthy adults as a control group. Sixty type 2 diabetic patients. All the studied individuals were subjected to full history taking and clinical examination. In the present study, we investigate the association of serum resistin levels with variable parameters: age, BMI, fasting and post prandial blood glucose , blood urea, serum creatinine, glycated Hb, serum insulin, insulin resistance by HOMA-R, - cell function by HOMA-B and insulin sensitivity by QUICKI. The result of the present study revealed non significant differences in serum resistin levels between diabetics and controls. There were no statistically significant difference between males and females in both diabetics and controls as regard serum resistin levels. There were no statistically significant difference as regard serum resistin levels in normal weight, overweight or obese diabetic subjects. There were no statistically significant correlation between serum resistin levels and all the studied parameters in both diabetics and controls. In conclusion, the results of the present study in keeping with evidence from literature revealed that resistin may not be involved in the pathogenesis of insulin resistance, obesity and / or type 2 DM. However, further studies on a wider scale for further evaluation of the relation of resistin to the components of metabolic syndrome.
