Bleeding oesopheal varices is one of the major healthproblems in Egypt.· Untill now openion of the most effectivemethod in the management of the acute attacks is confused. EEAstapling is a relatively simple and qutck operative method. thathas been reported to have a high efficacy in prevention andcontrolling variceal bleeding . In our work a trial was carried outto study the effects of EEA stapling on 30 patients withoesophageal varices with history of bleeding . EEA stapling wasfound a simple . safe and efficient method in controlling thevarices and prevention of vartceal haemorrhage . there was noserious complications and no apparant embarrassment to theliver function directly related to the technique. Also theoperation does not prevent any further surgical interference inthe form of any of the portasystemic shunt operations Inaddittcn.the rate of recurrence of bleeding after the operation isvery low.In conclusion , oesophageal transection with splenectomyand devascularization may be recommended as the definitivemethod of choice in good risk patients (Child A or B). Thisrecommendation awaits the outcome of further long termcontrolled clinical trials that include and focus on the mostappropriate therapy for individual patient with bleedingoesophageal varices. |
