Benign prostatic hyperplasia (BPH) is a urologic disease that affects males 50 years of age and older. It is a common disease of the elderly that has rapidly increased in incidence with the growing elderly population, economic growth .In selecting patients for TURP, predicting the persistence of irritative bladder symptoms is important, but studies investigating the prognostic factors of bladder irritation are uncommon. Urodynamic are used to assess the presence of detrusor over activity, bladder outlet obstruction and detrusor contractility preoperatively to predict the improvement of over active bladder symptoms after TURP . An increasing degree of preoperative bladder outlet obstruction has been associated with increased symptomatic efﬁcacy of TURP .
Patients and methods :This study is a prospective randomized study that was conducted on 150 patients with BPH and OAB symptoms, from Augast 2017 to Augast 2018 in urology department, Benha university hospital. all patients had BPH and OAB symptoms and were indicated for TURP Patients were divided according to urodynamic pre operative in to two groups: Group (A) included 102 patients without detrusor overactivity.Group (B) included 48 patients with detrusor overactivity. We assed predictors of over active bladder symptoms improvement after TURP like:Age ,Over active bladder symptoms score (Blaivas score2007) and urodynamic parameters Results :Overall, there was a statistically significant improvement in mean overactive
bladder symptoms score from18.97 to 9.14 . we found that number of patients with persistent over active bladder symptoms37 (24,66%) and number of patients with resoluted OABS were113 (75,34%). The significant parameters are age, Prostate size , IPSS, Q MAX ,MCC ,BCI and Terminal detrusor overactivity.
Conclusion: we concluded that older patient(70years old) with more prostatic size(61gm) and less IPSS (24) score, high QMAX (8ml/sc),low MCC
